Payment

• When due:
  FULL PAYMENT is due at the time of service for ALL patients.
  
  For treatment of minors who are not accompanied by an adult,  pre-authorized charges to a credit plan or a credit card can be arranged.
   
• Deductible & non-covered expenses:
  If you have insurance but no secondary or supplemental insurance, you must pay at time of service either:
   
  •  the co-insurance percentage of total charges
        OR
  • amount of non-covered charges
       plus any unpaid deductible,  if known
        OR
  • the co-pay amount
     
• Payment methods accepted:
  • Cash
  • Checks
  • 
    VISA, MasterCard, Discover, American Express
