YOU are ultimately responsible for the timely payment of your
account.
Your insurance is a contract between you and your insurance company. We
are not a party to this contract! We file claims as a courtesy to our
clients. We have agreements about claim-processing with some companies.
We will make every effort to:
- Verify your insurance.
- Check our status as to being a preferred provider with your
insurance company.
- Pre-certify or get prior authorization, when called for by your
policy.
We will NOT become involved in disputes between you and your insurance
company, other than to supply facts. You are responsible for knowing what
your insurance does and does not cover.
When payment is not made:
- Insurance fails to pay:
If your insurance company refuses to pay or has not paid after 45
days, your bill will be turned over to you for immediate payment. You
will have 15 days to pay the balance without a late fee.
- Late fees:
1.5% monthly for late payments after 30 days from date of service
Specific insurance situations:
- Medicare - YES
We DO accept assignment under Medicare. Co-insurance charges or
denials are the responsibility of the insured. If you have more than one
insurance policy, you must inform us which is primary and complete a
Medicare Secondary Payer Form.
- Worker's Compensation - YES
If your worker's compensation claim is denied, you are personally
responsible for your bill.
- Champus/Tricare Insurance - NO
We do NOT accept Champus or Tricare Insurance, nor do we accept
assignment, nor do we accept their allowed amount for services. If you
have Champus or Tricare Insurance, you will have to pay 50% of charges
at the time or service, even if you have supplemental insurance. As a
courtesy, we will bill Champus/Tricare for you but, as a general rule,
the check will go to you instead of to us.
- Auto Insurance - YES
Any charges not paid by medical pay provisions of auto insurance
(yours or that of other parties in the accident) are YOUR responsibility
and are due and payable in full upon receipt. We WILL NOT file a Motor
Vehicle Accident (MVA) claim with a private health insurance plan.
