Your First Visit

We want your first visit to be pleasant and productive, so please come prepared:

• Please arrive 15 minutes before your scheduled appointment to complete the paperwork.
   
• Plan to be here for at least an hour.
   
• Bring:
  • Your most current insurance card
  • Copy of  any other insurance information
  • Doctor's prescription for therapy
     
• Your physical therapist will:
  • Collect detailed medical history.
  • Evaluate possible therapies.
  • Consult with you and your doctor about a treatment plan.
  • Verify your insurance coverage.
     
• If your insurance allows, you may be able to begin treatment at this visit.
